The amount of times I have had that happen is insane...I had my party kill a friendly creature before. It spoke telepathically, this freaked them out and they killed it..He was being friendly too, rofl there was no overt threat besides he looked strange.

Players are conditioned this way.

Cloaked stranger in the corner of a tavern: He needs confronted or followed.

NPC coughs in-character: Plague.

Random person met by side of road: Agent of evil or god/dragon in disguise.

Another party met who keeps their wagon covered: Gotta get the Rogue to search that.

First pretty girl to talk to male PC anywhere: Sleep with her!
